How to Create a Form in Notion

Steps

  1. Type /form on a blank line in your Notion page to trigger the block menu.
  2. Select Form from the block menu to insert the form builder.
  3. Add a Title for your form in the form builder.
  4. Add new questions and choose the appropriate question type for each one.
  5. Click Share form at the top of the form builder to open sharing options.
  6. Under Who can fill out, choose who can access the form.
  7. Copy the form link and share it with your audience.
  8. Click Preview at the top of the form builder to check how your form looks.
  9. Your Notion form is now ready to collect responses and keep them organized in your workspace.
