Here’s How to create a form in airtable:

1. Click on the ‘Views’ option available in the header.
2. Click on the ‘Form’ option in the bottom-left corner under ‘Views’.
3. Enter all the details of the questions you’ll need in the form.
4. Once it’s done, click on the ‘Submit’ option and if you need you can alter the settings available below.
