Use this interactive demo to learn how to rename a form in Microsoft Forms.

‍

‍

Microsoft Forms lets you easily rename by editing the title directly, perfect for organizing surveys by date, topic, or client. Follow these steps to set it up:

‍

  1. On your Microsoft 365 main dashboard, click the App launcher (grid icon) in the upper-left corner.
  2. From the app list, select Forms to open Microsoft Forms.
  3. Click on the specific form you wish to rename to open the editor.
  4. Click the title area at the top of the form, then type your new form name.
  5. Click Preview to confirm the change appears correctly.
  6. The new name will now appear on your dashboard and at the top of the form for respondents.

📌 Why this matters

Renaming forms keeps your workspace organized and makes it easier to find specific surveys later, especially when managing multiple forms across different projects or time periods.

Beyond basic organization, descriptive form names appear at the top of each survey for your respondents, creating a more professional first impression and helping participants understand exactly what they're completing. Clear naming also becomes crucial when sharing forms with colleagues or when forms are embedded in different contexts where generic titles would confuse users.
