  1. On your Microsoft 365 main dashboard, click the App launcher (grid icon) in the upper-left corner.
  2. From the app list, select Forms to open Microsoft Forms.
  3. Click on the specific form you wish to duplicate.
  4. Hit the three dots menu to access more form options.
  5. Select Collaborate or duplicate from the menu.
  6. Click "Get a link to duplicate" to generate a shareable template link.
  7. Click "Copy" and you're all set—share it with your team!
