Use this interactive demo to walk through the steps of creating a sign-up sheet in Microsoft Forms.

Microsoft Forms allows you to easily create sign-up sheets to manage events, appointments, or volunteer lists. Follow these steps:

  1. On your Microsoft 365 main dashboard, click Apps to see all apps.
  2. Click on Forms to open Microsoft Forms.
  3. Select New Form to start a blank form.
  4. At the top, click “Untitled form” and fill in your poll title and description.
  5. Click Quick start with, select the Text question type, and create fields for Name, Email, Phone number, or other details you want participants to provide.
  6. Toggle Required for fields that must be filled out.
  7. Use the Style button (paint palette icon) to customize the appearance of your sign-up sheet.
  8. Pick a theme or color scheme that matches your brand.
  9. Click Collect responses to share your sign-up sheet via link, QR code, email, or embed code.
