Microsoft Forms is an excellent tool for gathering feedback, conducting research, or running surveys. Follow these steps to build a professional questionnaire:

  1. On your Microsoft 365 main dashboard, click Apps to see all apps.
  2. Click on Forms to open Microsoft Forms.
  3. Select New Form to start a blank form.
  4. Click "Untitled form" at the top and enter a descriptive title and objective.
  5. Click Quick start with and select Choice for multiple-choice questions or Text for open-ended answers.
  6. You can also select from the available templates on the left side of the page
  7. Choose the Customer Satisfaction Survey to load a pre-designed layout.
  8. Toggle the Required switch for any questions that are mandatory for your data.
  9. Use the Style button to change the background or upload a logo to the header.
  10. Click Collect responses to generate a link and distribute it to your customers.
