Here's how to do it:

1. Navigate to Pages > Add New (or Posts > Add New).
2. Click the + sign on the upper left corner.
3. Search for Forms and click on it.
4. Choose a template for your form.
5. Customize your form accordingly.‍
6. Save your changes and publish the page or post.
