  1. On your Microsoft 365 main dashboard, click the App launcher (grid icon) in the upper-left corner.
  2. From the app list, select Forms to open Microsoft Forms.
  3. Click the New Form button on the Forms homepage.
  4. Fill in the required details, then add your questions, choose the question type, and mark them as required if needed.
  5. Use the Style option (paint palette icon) to customize your form’s design.
  6. When done, review your form and make any necessary edits before clicking Collect responses.
