Use this interactive demo to learn how to create a feedback form in Microsoft Forms.

‍

‍

Microsoft Forms is a powerful tool for capturing SaaS user feedback, product reviews, or customer satisfaction data. Follow these steps to build your feedback form:

‍

  1. On your Microsoft 365 main dashboard, click Apps to see all apps.
  2. Click on Forms to open Microsoft Forms.
  3. Select New Form to start a blank form.
  4. Click "Untitled form" at the top and enter a descriptive title and objective.
  5. Click Add new and select Rating to let users quickly score their overall experience.
  6. Click again on Add new question, then choose Net Promoter Score® from the menu to measure user loyalty.
  7. Once you have finished adding your questions, click Style to customize your survey's appearance.
8. Click Collect responses to access sharing options and start gathering feedback.
  9. Click Copy link to grab your survey URL and share it with your audience.

📌 Why this matters

Creating feedback forms transforms scattered opinions into actionable data you can track over time. You'll capture both emotional responses through ratings and loyalty metrics through Net Promoter Score, giving you a complete picture of user sentiment.

The real advantage lies in Microsoft Forms' integration with your existing Office 365 ecosystem - responses automatically flow into Excel for analysis, sync with Power BI for dashboards, and connect to Teams for instant notifications when critical feedback arrives, creating a seamless feedback-to-action workflow.
