Microsoft Forms is an excellent tool for gathering feedback, conducting research, or running surveys. Follow these steps to build a professional questionnaire:

  1. On your Microsoft 365 main dashboard, click Apps to see all apps.
  2. Click on Forms to open Microsoft Forms.
  3. Select New Form to start a blank form.
  4. Click "Untitled form" at the top and enter a descriptive title and objective.
  5. Click Quick start with to speed things up with pre-built question templates.
  6. Or, click Choice to add a multiple-choice question.
  7. Toggle the Required switch to ensure the user cannot skip the checklist section.
  8. Use the Style button to customize the visual style or add a company logo.
  9. Click Collect responses to get your shareable link and start gathering feedback.
  10. Click Copy link and share it with your audience—email, Slack, wherever works.
