While Microsoft Forms doesn’t offer a native "drawing pad" for signatures, you can easily create a legally-binding digital consent using a Choice question.
- On your Microsoft 365 main dashboard, click Apps to see all apps.
- Click on Forms to open Microsoft Forms.
- Open the specific form for which you wish to create a Sinature.
- Click Add new question to expand your form with another field.
- Select Choice to let respondents pick from predefined options.
- In the Question box: Enter the legal declaration or consent statement.
- In 'Option 1': Type a clear affirmative statement. Example: "I formally accept and agree."
- Delete 'Option 2': Click the trash can icon next to the second option so only one choice remains.
- Toggle the Required switch so students or employees cannot skip the matching section.
- Click Collect responses to generate a link and distribute it to your participants.
