How To Add Signature In Microsoft Forms

Use this interactive demo to learn how to add a signature to your Microsoft Forms.

While Microsoft Forms doesn’t offer a native "drawing pad" for signatures, you can easily create a legally-binding digital consent using a Choice question.

‍

  1. On your Microsoft 365 main dashboard, click Apps to see all apps.
  2. Click on Forms to open Microsoft Forms.
  3. Open the specific form for which you wish to create a Sinature.
  4. Click Add new question to expand your form with another field.
  5. Select Choice to let respondents pick from predefined options.
  6. In the Question box: Enter the legal declaration or consent statement.
  7. In 'Option 1': Type a clear affirmative statement. Example: "I formally accept and agree."
  8. Delete 'Option 2': Click the trash can icon next to the second option so only one choice remains.
  9. Toggle the Required switch so students or employees cannot skip the matching section.
  10. Click Collect responses to generate a link and distribute it to your participants.
