Steps

  1. In the Design Survey tab, go to the Build Section and click Add next to the Single Textbox option.
  2. Type a statement in the text field that explains what the respondent is agreeing to by entering their signature.
  3. Select Options from the question box to open the question settings.
  4. Set the question to require an answer so respondents cannot skip the signature field.
  5. Click Save to apply the question settings.
  6. Click Preview Survey to verify how the signature field appears to respondents.
  7. Confirm the completed setup — your signature has been successfully added to SurveyMonkey.
