Steps
- In the Design Survey tab, go to the Build Section and click Add next to the Single Textbox option.
- Type a statement in the text field that explains what the respondent is agreeing to by entering their signature.
- Select Options from the question box to open the question settings.
- Set the question to require an answer so respondents cannot skip the signature field.
- Click Save to apply the question settings.
- Click Preview Survey to verify how the signature field appears to respondents.
- Confirm the completed setup — your signature has been successfully added to SurveyMonkey.
