How To Add a Logo in Microsoft Forms

Adding a logo is essential for making your SaaS forms look official and branded. You can place your logo at the very top of the form next to the title. Follow these steps to set it up:

  1. On your Microsoft 365 main dashboard, click Apps to see all available tools.
  2. Click on Forms to open Microsoft Forms.
  3. Open the specific form where you want to add your branding.
  4. Click on the Form Title area at the top of the page.
  5. Click the Insert Image icon (the small picture frame) on the right side of the title box.
  6. Select Upload to choose your logo file from your computer (PNG or JPEG works best).
  7. Click Preview to see how your logo appears to respondents on both mobile and desktop.
  8. Your logo now appears prominently at the top of the form.
