1. On the main menu, click Design Studio.
2. Click New, then click New Form.
3. Enter your Form’s name and description, then click Create.
4. Fill up your Form Details, then click Next.
5. Continue working with your Form Settings, then click Next.
6. Once everything is complete, click Approve and Close.
