Steps

  1. Go to Forms.app and click the three dots under the form you want to connect with Trello.
  2. Select Edit to open the form settings.
  3. Click Connect on the form dashboard.
  4. Search for Trello in the integrations list and click Manage.
  5. You will be redirected to Zapier — log in and click Connect these apps.
  6. Click the drop-down menu to choose a trigger event for Forms.app.
  7. Select the specific form you want to connect with Trello.
  8. Click Continue with selected record to proceed.
  9. Select a Trello action event from the available options.
  10. Choose the Trello board you want to integrate the form with.
  11. Fill in the required fields — including value, file attachments, and URL attachments — then click Continue.
  12. Click Publish to activate the Zap and complete the setup.
  13. Go to your Trello account and open the board you selected in Zapier.
  14. Click the Power-Up icon at the top of the board.
  15. Click Add Power-Ups to open the Power-Up directory.
  16. Search for Zapier in the Power-Up directory and click Add.
  17. Click Add again to enable the Zapier Power-Up for your selected Trello board.
